DULUTH — Minnesota officials have found 22 licensing violations at a Duluth methadone clinic already operating on a revoked license.
State documents released Tuesday show the Lake Superior Treatment Center was cited for violations discovered during a review earlier this year.
Keith Koegler of the Minnesota Office of Inspector General says the citations are “serious and substantial” in a letter dated Nov. 6 to Florida-based Colonial Management Group, which owns the treatment center for narcotics addition.
Alleged violations include unauthorized disclosure of client records, improperly giving out methadone for take-home use and not keeping proper documentation.
